Question: I fell on 7/24/16 and went to emergency, I badly bruised my right leg but x-ray showed no break. However it is very painful and swollen. I have been elevating it and applying ice. However when I saw my doctor she said not to ice it. just wrap with ace bandage and elevate. thanks for any advice you can give me.

Brief Answer:
Anti-inflammatory Rx and investigations as advised.

Detailed Answer:
Hi.
Thanks for your query and an elucidate history.
Read your history and understood the concerns.
To recapitulate: Female/91 - fell on 7/24 - ER - normal X-ray - bruised right leg - it is painful and swollen - elevating and icing - Dr said no to ice, wrap with ace bandage and elevate - pull out blood but did not wok - wants advise ...

Since it is almost 2 weeks and the pain and swelling have still persisted, means there has to be a reason.
The most probable causes can be:
Hematoma not resolving but organized now (needle could not draw out the blood).
tear of the tendon/muscle
Fracture
dependent edema

I would advise you the following:
Review X-ray to see for the missed hairline fracture.
MRI too is a good investigation.
Ultrasonography to see if there is an edema or a clot/hematoma. Color doppler mode to see for any vascular abnormality, varicose veins or incompetent venous valves.
Add on an anti-inflammatory medication with painkiller property like Ibuprofen as a PPI like Omeprazole to control the acidity that an anti-inflammatory medicine can cause.
Keep the leg elevated.
Apply ace bandage.
Further management will be as per the reports of the investigations.
Please discuss this with your Doctor and insist on investigations so that the most proper diagnosis and treatment can be planned.

I hope this answer helps you, please feel free to ask for further relevant queries if you feel that there is a gap of communication.
