I have gone through your question, reports and understand your concern; it suggests that your son may be suffering from the chronic tonsillitis with adenoid facies with otitis media.
In this condition, if I were the treating doctor then I would like to suggest that your son may go for the tonsillectomy with adenoid removal without grommet insertion.
According to my best of knowledge for ear surgery your son may wait till 10 years of age.
After tonsillectomy and adenoid removal and with conservative management, ear condition may improve without the surgery; therefore better to wait till the age of 10 years for grommet insertion.

Hi Sir,
I have gone through your follow up query; and you are most welcome.
I would like to explain the treatment procedure as below
1. Immediate response - Go for the tonsillectomy (remove the tonsils) with adenoid removal (adenoidectomy). Both procedures are often performed at the same time and for your knowledge the tonsils are two clusters of tissue located on both sides of the back of the throat. Adenoids sit high in the throat behind the nose and the roof of the mouth. When they become enlarged and block the upper airway, leading to breathing difficulty.
2. After surgery your son may go for the conservative management instead the grommet insertion.
3. If condition of the ear persists even after the 10 years of the age then go for the grommet insertion. For your knowledge Grommets, also called tympanostomy or ventilation tubes, are tiny plastic tubes which are inserted into a small slit in your child's ear drum in a short operation.
