Brief Answer:
Wound swab testing/culture advised

Detailed Answer:
Hi back my dear,

Ok, I've gone through your photo and noted that swollen second toe. As keflex and cipro failed to treat it, and from what I have seen, that's a case of fungal infection of the skin of second toe with secondary bacterial infection.

A further culture of wound discharge/swab is also advised as next diagnostic step.
Examination of swab for fungi is also recommended

Antifungal treatment is required orally and locally with terbinafin cream. Keep it well clean and dried with terbinafin powder.

Raise your leg to help circulation of blood.

Hope this helps.
Dr.Albana
