Brief Answer:
Lichen Sclerosis et Atrophicus

Detailed Answer:
Hi.

Thank you. I have reviewed the Images.
I would keep a possibility of lichen sclerosis et atrophicus (LSeA) rather than a consequence of fungal infection.
I suggest you to use a moderately potent topical steroid e.g mometasone furoate 0.1% cream, twice daily.

Regards

Will this improve the appearance? My urologist will not reconsider his diagnosis. Have called him twice in the past two weeks.
Do I need a prescription for this steroid cream?

Brief Answer:
Mometasone furoate is a prescription steroid

Detailed Answer:
Hi

Yes. This cream should help.
Mometasone furoate is a prescription topical steroid. You should contact a dermatologist for a prescription.
An alternative to topical steroid is topical tacrolimus ointment. It is also a prescription product.

Is it common for the skin to turn white over such a large area in lichen sclerosis?

Brief Answer:
LSeA can be extensive

Detailed Answer:
Hi.

Yes, LSeA can involve a large part of genital mucosa. LSeA can also involve extragenital skin. Involvement varies from individual to individual.
You may use either mometasone furoate 0.1% ointment or tacrolimus 0.1% ointment, 2x daily for a few weeks.

Regards
