Brief Answer:
tramadol dose can increase, nsaid can change, add paracetamol

Detailed Answer:
HI, thanks for using healthcare magic

Currently you are using an opiate (the tramadol) and an NSAID ( the diclofenac).

The tramadol dose can be increased unless there is a medical reason that you cannot take the maximum dose. The maximum dose is 400mg a day which is 100mg every 6 hours ( 4 times daily)
This would be associated with increased drowsiness.

If diclofenac 50mg three times a day is not enough then you can switch to celebrex 400mg which is reported to be stronger or add another medication.

In terms of additions- paracetamol/tylenol can be added since it is not in the drug families mentioned (opiate like tramadol or nsaid)
This can be taken 1000 mg every 6 hours.

If the increase in tramadol and addition of paracetamol is not enough then as suggested, the medications would need to be changed.

You would need a different opiate (codeine, hydrocodone etc) or different NSAID eg ketoprofen or celebrex.

Should not take 2 nsaids together eg diclofenac added to ibuprofen or naproxen, need to switch

I hope this helps, feel free to ask any other questions
