Question: I have had left arm pain since smoking three hours ago. No chest pain or shortness of breath. What could be causing the pain? The pain goes in and out but hurts badly.

I appreciate your concern

It can be musculoskeletal related pain. However, a cardiac cause must be ruled out especially if there is no response to the ibuprofen as any sprained or sore muscle should respond to the ibuprofen. Get an EKG done to look for any abnormality. If normal, NSAIDs and muscle relaxants can be advised. Smoking is a known risk factor for coronary artery disease and a bunch of other disorders. Try to quit smoking gradually. If difficult you can consult a doctor for prescribing bupropion to help you quit smoking with milder withdrawal.
