The most common cause of cough/cold and fever in this age is some viral infection.
Nasal blockage and noisy breathing is very common in this condition.
Sinusitis is not common as sinuses are not well developed in this age.
I would suggest you to give him combination of antiallergic like chlorpheniramine and decongestant like phenylephrine(like Drop maxtra).It is a prescription medicine so please get it prescribed by a physician.
It helps in relieving cough and also nasal blockage.Continue giving nasal saline drops.
As fever is not of very high grade chances of bacterial infection is very less and so there is no need of antibiotic.
Try to give frequent feeds with plenty of fluids to maintain the hydration.
It may take 4-6 days for improvement in symptoms.
Observe for any breathing difficulty,excess irritability or decreased urine output (suggestive of poor feed)and if you notice any consult your pediatrician.

Decreased urinary frequency indicates poor hydration due to poor oral acceptance.
I suggest you to give him frequent feeds and observe for urinary output.If urinary frequency is less than 3-4 times in 24 hours he may need to get admitted with Intravenous fluid supplementation.
The dose of maxtra for this age is 1 ml three times a day.
Watch for respiratory distress and if you find any abnormality consult your pediatrician immediately.
