Suggest Remedy For Irregular And Slow Heart Rate

Question: Hi I have had all cardiac tests for the second time approximately 1 and a half months ago. I had ecgs, 48 hour holter monitor, echocardiogram and stress tests as well as blood work. All my blood work seemed normal. The only thing I noticed is my b12 level was in the low range of normal 217. My echo showed a structurally normal heart with some trace regurgitation. My stress test was normal according to my cardiologist. My holter showed 150 PACS and 15 isolated PVC. I had bradycardia 26 % of the time and tachycardia 12 % of the time. My lowest heart rate was 41 during sleep and average heart rate of 70. My resting heart rate seems to be between 54-60. My cardiologist says this is all normal and my heart was in sinus rhythm no arythmia detected.

I notice sometimes throughout the day my heart will feel irregular. Sometimes I will get some slower heart beats for 7 or 8 beats which I can feel in my throat. These are not pvcs they feel like normal beats a d then my heart will beat normal again. I do have some acid reflux and I'm not sure if this is being caused by the vagus nerve. When I wake up in the morning my heart beat will also feel irregular. I'm not sure what is causing it but I've had so many tests and everything seems to be normal. I had a loop monitor 2 months ago and again my cardiologist says it was normal. Please help!

Brief Answer:
Nothing worrisome noted as described below...

Detailed Answer:
Hi,

I have gone through your medical history carefully and understood your concerns.

1. You are reporting to be suffering of acid reflux/GERD. This is triggering heart palpitations because heart and stomach and esophagus are close to each other.

2. You are also reporting to have already run Holter monitoring and resulted with isolated PVC and PAC'S. They are within normal range for 24 hours period and would make you feel more irregular heart beats. However, they are benign and not worrisome.

3. With regards to vagal dystonia, I can say that you should get it determined first. I admit that vagal dystonia and other vagal problems would aggravate/trigger symptoms related to acid reflux and palpitations.

Once again, although such explanations, I admit that there is nothing serious noted going on with your health.

All the best!

Hi Doctor.

I had another episode today where my heart started beating quickly and they were forceful beats. Even as it was slowing down I could feel the beats were still very strong. Is this common with anxiety. Also is it dangerous for the heart to beat at 160 BPM and strong during anxiety. Does it damage the heart. Usually the episodes only last a few minutes

Detailed Answer:
Hi back,

What you have experienced is very common with anxiety. This isn't dangerous even if your heart beats very fast (160 bpm).

Tachycardia is also a common finding associated with anxiety and it can't cause any damage to your heart.

Wish you health!

Hi Doctor Thank you for your response. My holter showed XXXXXXX R-R of 1.5 seconds at 5 AM during sleep. Is this a sinus pause and is it normal?

Following answer to your query.

Detailed Answer:
Hi back,

It doesn't show a sinus pause, it shows that the lowest HR was 40 BPM at 5 AM during sleep (heart rate is calculated as 60/ R-R interval in seconds).

It's normal to have low HR in the early hours of the morning and you shouldn't worry about that.

Wish you health!
