Brief Answer:
Could be Atypical cardiac pain

Detailed Answer:
Hi,

Thank you for your query. I can understand your concerns.

Cardiac chest pain or anginal pain may radiate to left arm and left shoulder. These places of reference may be involved without discomfort in the chest. The relationship to exertion is then the indication that the pain is probably of cardiac origin. It is then called atypical cardiac pain.
Since smoking is a strong risk factor for coronary artery disease ,I would advise you to undergo ECG and stress test to rule out ischemic heart disease.
