You should get XRay of the Chest AP and lateral views as well as lateral view of your shoulder and arm. If they are normal then a CT of the chest might help to know what has happened as it can also show the shoulder joint.

You should start Respiratory breathing exercises or start Incentive Spirometry with a Spirometer. This will help you regain your lung capacity.

Without examining you it is very difficult to say what it is. Try to get the above tests done and show it to a Surgeon. You could change your Doctor perhaps. Stronger pain medicines will also help like Fenatnyl patches.
But you need to get the CT done to know what is going on.
