Suggest Remedy For Painful Periods When On Logynon

Question: Hi. I just have a query relating to the pill logynon. I have been on this pill for over 10 years and have never had a break. I suffer with agonising period pain so I use the pill to help with that. I was also diagnosed with Crohn's disease 4 years ago. I had a colonoscopy recently which showed I am in remission still and it is not active. However, even though the pill was very effective for period pain up to about 6 months ago my period pain has become worse each month. My doctor suspects endometriosis and I'm waiting for an appointment with a specialist but that will take a few months. In the meantime, he told me to take the pill back to back for 3 months. I did this but on the second month got break through bleeding. By the third month I bled for a whole week nearly even though I was still on the pill. Then I had my break through bleed when I finished the pill which was horrendously painful. After my 7 day break I went back on in the pill this month but have suffered terrible pain throughout the month at various times. Did taking the pill back to back upset my cycle completely? Will this settle over time or is this pill just no longer effective? I feel by taking the pill back to back things have been made way worse. Have also been put on Effexor for anxiety in the last 5 months too. Not sure if this may have had an effect on my periods too? Many thanks. XXXX

Thanks for the query to hcm..
You have been using logynon which is a combined oc pill for last 10 years and have been experiencing dysmenorrhoea or painful menses lately and also heavier since last 3 cycles.
Firstly oc pills if used prolonged can often loose their efficacy in itself,not the contraceptive part but other positive effects like flow control and dysmenorrhoea.Secondly as you have been on effexor which is an antipsychotic drug and us being uaed daily since last 5 months when yout symptoms actually started is the prime cause of decreased efficacy if your pill itself. Antipsychitics and antianxiety pills interact with hormone pills and render them with decteased efficacy and hwnce you may land up experiencing break through bleeds in the middle of the cycle with increased blood flow and pain.
This is what has happened with you. Also the possibility of endometriosis or adenomyosis needs to be ruled out which can get potentiated due to irregular bleeding since last 6 months and can be a potential cause of dysmenorrhoea itself. But the prescription for continuous use of logynon ,a combined estrogen progesterone pill is not correct and it would give you heavier breajthrough and also hevy menses. For endometriosis progesterone only pills like cerazette, should have been prescribed which would have caused amenorrhoea and reduced the endometriosis and the pain..if at all its suspected.
So I feel you need a rescheduling of your pills and decreasing your effexor dose as well which shall finally help you.

Thank you for your feedback. I asked my psychiatrist about the effect Effexor could have on my periods but he told me it may increase bloodflow but would be surprised if it would cause anymore pain. He suggested better pain relief. That is when my own doctor suggested putting me on logynon back to back for 3 months. My periods have been getting bad again for about the last year but the last few months have been like it was before I took the pill - agonising! It looks quite likely that the Effexor is effecting the effectiveness of the pill with regard to pain relief. However I found it very difficult to find an anti depressant which suited me due to the Crohn's. I'm on a low dose 37.5mg and my mental health is much better. I will definitely talk to my psychiatrist about this however. Is the pain I have this month on the pill mid cycle which I never ever get a result of taking logynon back to back for 3 months or the Effexor do you think? Should I also now change my pill or should I wait to see the specialist and rule out endrometriosis first? Thanks again.XXXX

Hello,
I shall list it out for you:-
1. Meet the specialist and rule out endometriosis and adenomyosis with ultrasound pelvis.
2. If your concern is contraception alone, then you may opt for IUD, but if the pills were for heavy flow then you have to either stop effexor or change the antipsychotic completely with the pill.
3. Presently you may consider progesterone only pills for 3 months back to back which will not have oestrogen which can cause the present irregular shedding. Hence brand like CERAZETTE can be used till endometriosis is evaluated or ruled out.
4 If you are bleeding presently then you may opt for antifibrinolytics like tranexamic acid 500mg thrice a day for 3 days to reduce and stop the bleeding, this may be prescribed by your physician.
