You seem to be having severe arthritis of both the hip joints. This classically causes referred pain to the knees which is why your also have knee issues.

Usually there is no other go than hip replacement in the setting of severe hip arthritis.
Ankylosing spondylitis is an inflammatory disorder affecting the spine and hips causing stiffness and arthritis.
An Xray of the spine should be enough to make a diagnosis. It would be best to discuss with your treating surgeon about the benefits and risks of Joint replacement in your case.
Since you are already on strong pain killers, I can only suggest you take intramuscular pain killer like Diclofenac 1 ampoule once during a severe pain attack. This needs a prescription and needs to be administered by a trained health professional. The risks include pain in the region of injection, possible infection, possible nerve damage if administered in wrongly and also possible hypersensitivity reactions. Hence, please keep in mind the risks and kindly discuss with your Doctor for the same. The better methods of pain relief will be reduced weight bearing, use of walking aids. Kindly avoid Unprotected weight bearing or rather perform protected weight bearing with Walker, crutches or walking stick which will also help in the pain relief. Apply warm or cold packs, whichever relieves your pain.
