Read and understood that your brother has severe pain in his lower left back.
If there are no other associated symptoms related to the urine or stool, this may be be purely a local pain due to muscle and/or bone problems.
The symptoms of the kidney stones are:
Most the stones in the kidney proper do not cause any pain unless there is infection or impaction of the stone in the narrow part of the outflow of the kidney called pelvis going into the ureter or in the ureter or at the outflow of the bladder going into urethra.
Kidney failure can cause reduced or stoppage of urine production, swelling all over the body and so on.
I would advise in such a situation to get a clinical evaluation, actual examination and investigations like ultrasonography of the whole abdomen, tests of blood and urine, MRI of the back part for spine and muscle if required to get a proper diagnosis and plan for a proper treatment.
If the pain is more he can surely take painkiller which suit him.
He should drink normal or more than normal water.
