Posted on Mon, 13 Jul 2015
Question: Hello Doctor,

I have been suffering from one sided throbbing pain on the right side of my head from a couple of days. I have been experiencing this on/off from a month (this is the second time I am experiencing this in 30 days) but this time the pain is severe and it starts form above the eye until the back of my head.

If this is migraine - what tests should I get done to conclude on the problem?

Hello XXXXXXX
I have gone through your question and understand your concerns.
Your headache seems to be migraine like.
Migraine is a primary headache with features

2) hemicranial or holocranial
3) mild to moderate intensity
4) peaks in hours
5) associated with nausea or vomiting, photophobia and phonophobia

7) aggravates with chocolate, delay in meals
8) releiving to some extent with rest or pain killers
Danger signs for secondary headache are severe headache, vomiting, fever, visual loss, hearing abnormalities, speech difficulty, dysphagia or limb weakness, seizures.
Second most common headache is migraine after tension type headache.
For investigation, no particular test is available. Only history is suggestive. Any brain imaging is done if some other diagnosis is being suspected.
For treatment you can take pain killers for few days and later on prophylaxis of migraine may be needed if symptoms are persistent or involve most days of month.
Prophylaxis drugs are propranolol, calcium channel blockers, Topiramate, amitryptiline, Valproate.
For acute attack you may try sumatriptan or rizatriptan only in guidance of a doctor.
