Hi.
Thanks for your query and an elucidate history.
To recapitulate salient points: Female/32 - suffering for 5 years - cholecystectomy in 2009 - since then pain mimicking GB attack - dull and within minutes vomiting, shaking, sweating and hard to breathe because **the pain shoots through center chest into back - *constant dull pain under right rib cage - many tests and pills - told as IBS/Endometriosis - attacks 3 days before periods and during ovulation.
My thoughts on reading your history:
- Endomeriosis is suspected because your history says that attacks commonly come usually three days before the start of your period and then sometimes during ovulation.
But this can cause the edema at many places in the body as PMS and enhance the symptoms.
- This does not look like IBS as history lacks any intestinal symptoms.
- The history of pain shooting through the *center of chest into the back is suggestive of some nerve compression in the thoracic region.
- Severe pain can cause other symptoms you have noted like sweating, vomiting, shaking and hard breathing.
I would like to ask a few additional information:
- Was MRI of the spine done?
- Do you get other PMS symptoms like heaviness in the breasts and body, swelling over the body and so on?
- EKG and cardiac checkup as there is pain in the chest?
- Any other information which you may feel relevant or absolutely irrelevant that you would like to share with me.
Please give feedback on these points as these are self-explanatory.

Go for MRI of the thoracic spine.
Detailed Answer:
Thanks for your feedback.
Get the MRI of the spine and the answer may be there with presence of discs or Compression of some sort on the spinal cord or the roots. This gets enhanced due to PMS causing the edema and further compression at this area causing enhancement of your symptoms.
Get this done as early as possible.
Bowel thickening and normal EKG noted. Endometriosis causing constant pain under rib cage is not known and can not be seen during C-section.
Please tell me the reports of the MRI of the thoracic spine.

Please discuss with your Gynecologist.
Detailed Answer:
This could have been detected in your previous scans in the area of pains.
Now can be tried to see with diagnostic laparoscopy.
Your Gynecologist can help you more. Please discuss with Her/Him.
