Suggest Test To Get Accurate Result Of GFR Level

Question: I have a question about chronic kidney disease.I have had interstitual cystis for many years. This past week my doctor reviewed my recent blood work and said I was in stage 3 chronic kidney disease with a GFR of 47. I've never had trouble with my kidneys before, I am not diabetic and I do not have high blood pressure. I do take Advil for pain with fibromyalgia. Do you think this could be a false positive for my GFR? Thank you.

Brief Answer: It is possible Detailed Answer: Hi Thanks for posting your query on HealthcareMagic. A GFR of 47 comes in the category of stage 3 chronic kidney disease. It does have its limitations. Inulin clearance is the best test to measure GFR but is not employed because of specific reasons. The next best test is GFR from creatinine clearance. Now creatinine is not only excreted by glomerulus but also secreted by tubules and excreted from other routes. One way to check is to correlate its values with levels of serum creatinine. If the decrease in GFR is coinciding with raise in serum creatinine, then probably kidney function is getting affected. An ultrasound of the kidney, ureter, bladder may also be done. Advil and other NSAIDs can also cause chronic kidney disease. So you need to avoid them. At stage 3, annual monitoring of Hb, potassium, calcium and phosphate are required. Also get your parathyroid hormones levels checked once and repeat if its levels are altered. Hope it answers your query. Let me know if I can assist you further. Dr Vaishalee

Thank you for your detailed response. I will follow up with a nephrologist in the next 2 weeks. I have also just found out I have a UTI and have been given antibiotics for it. I hope to feel better soon.
