Suggest Treatmemt For 100% Blocked Artery
Question: Hi, My mother diagnosed blockage in her both side carotid artery in Oct. 2013 (First stroke). Her left artery was 90% blocked and right artery was 70% blocked. We went for endarterectomy (left artery) in May 2014, after surgery she was absolutely fine for next 48 hours, and next day she was standing and got another stroke and she fell down on floor and after that we did MRI and doctor told us that her right artery is now 100% blocked, her left part got paralyzed.
Surgeon says we can't do surgery for a 100% blocked artery, someone also told that if she remain paralyzed for 3 months from the date of paralysis it would be dangerous for her.
Will she be able to get out of paralysis ? How long it will take ? Is it necessary to go for another surgery now?
What would you suggest, I can forward reports. I would be thankful to you if you can guide me.

Thank you for posting your query.
I have noted the details of your mother's illness.
It is unfortunate that she got paralysis on the left side. It is true that carotid endarterectomy surgery can not be done in a case of 100% blockage. In retrospect, the surgery should have done when it was 70% blockage. The surgery is recommended for any blockage of carotid artery 70% or more.
At this stage, she would need physiotherapy. With regular physiotherapy exercises, her strength would gradually improve, however, it would not become normal.
She should also continue anti-platelet agents such as aspirin/clopidogrel and statins such as rosuvastatin, to prevent further strokes in future.

For your reference & detailed study I have attached latest reports -
1. Latest Color Dopler report done on 21 XXXXXXX 2014 (Color Dopler - 21 XXXXXXX 2014)
2. Previous Carotid dopler reports done at XXXXXXX hospital (3files, Carotid dopler main, left & right)
3. Discharge summery and treatment
I would love to know -
1. Will that 100% blocked artery come to below 100% blockage or now it will remain 100% blocked long life ? If yes then what's approximate time frame (I am we should prepare ourself for that duration and can plan our further action as all family members are at job)
2. Secondly what would you suggest for second surgery? I mean if that blocked artery come to below 100% then it is suggested to go for 2nd surgery or we can leave it as I read on various sites that brain get blood through 2 alternate veins and also through clear artery. (This was her second stroke, first was in Oct. 2013 and then she back to normal status after medicine, but doctors asked us to go for surgery and then we went for surgery, and immediately after surgery she got 2nd stroke within 48 hours of surgery)
3. Any time frame you would like to suggest to get out of paralysis (a physiotherepist is taking care of her on daily basis)
