Usually the if root tips are left behind make sure you are under the cover of broad spectrum antibiotics like (Amoxycillin and Potassium clavulanate ) 375mg thrice a day for 7 days if you are not allergic to it. If you are already taking antibiotics then don't worry.
You can also take antiinflammatory medications consisting of (paracetamol 325mg and ibuprofen 400mg) twice a day for three days.
Kindly visit a dental surgeon or an orthodontist . After two weeks kindly get it removed.
Sometimes if root tips are left off and they dont cause problems then it can be left as it is. However, chances of infection exist hence it is better to get it removed. The dentists make a small incision and remove it if it doesnt get extracted easily .

There is a fear that in trying to remove the left out premolar root the sinus region may be in problem which may require more invasive treatment.

It is good that you are under the cover of amoxycillin and metrogyl(metronidazole).
During extraction of teeth the sinus may get exposed in some cases but your dentist shall definitely take care while extracting. They might take x rays to ensure the position of reminant root tip so that it is extracted out easiy. Usually sinus complications are seen in molar tooth extraction.
Since this is premolar donot worry.
After tooth root tip extraction be careful while chewing , coughing and sneezing to avoid pressure in that area.
Even if during extraction if sinus gets exposed, dental surgeons try to close it with side flaps or sinus lift procedures. Donot worry relax. Everything will be fine.
