Question: Hello Doctor, my daughter is 4 years old and has been having wheezing problem from quite some time. The issue started mainly after she turned 2 and has been constantly reoccuring till date. This generally starts with blocked nose, sneezing, stuffy and runny nose followed with severe cough and wheezing. Her treatment started with medicines like Tminic cold syrup, Darolac XXXXXXX Cefuroxime 125 mg/5 ml initially then Montair LC, Kofarest syrup, Otrivin nasal spray, Augmentin DDS, Crocin. Later the medicines included Alerid, Bricarex A and nebulization with Asthalin along with Augmentin DDS. Since yesterday the same issue started again and her treatment now is : Flutiflow Nasal Spray - once daily for 2 months T. montair LC - once daily for 10 days Syp. Bricarex A - twice daily for 6 days Nebulize with : Budecort 1 mg + Duolin - twice daily for 2 days then move to 1/2 Duolin - twice daily for 3 days. The issue of clogged/blocked nose is very frequent with her and use of Nasivion nasal drops/ Otrivin nasal spray is every now and then. As far my family medical history is concerned, no one has asthametic conditions. With my daughter being only 4 now and is being on such medicines so frequently i am a little affraid about her growth and permanent cure of her chest/wheezing/nose block issues. Please suggest a way forward. Kind Regards, XXXX

Brief Answer: Do not use otrivin repeatedly Detailed Answer: Hi Thanks for writing to health care magic. 1) Nose. Frequent running and clogged nose for the past 2 years makes the diagnosis of allergic rhinitis more likely. Flutiflo is an appropriate medicine for that which you can use as nasal spray daily. Make sure this is given every day. It takes 6 to 24 months, depending on severity and regularity of use. Do not use otrivin nasal spray repeatedly. this medicine by itself causes more clogging - Instead use steam inhalation and saline drops for nose when it is clogged. 2) Lungs . Your kid seems to have allergic bronchitis . I usually start with monteleukast but if no improvement , I usually start with inhalers like budesonide which is a steroid , twice daily 100mcg per puff. Do not worry. There are no side effects and it is the only type ie Steroid INHALER which will cure her. Whenever she coughs or has wheezing give her salbutamaol or duolin . Try getting allergy test done to identify the cause of allergy , so that you can avoid those triggers. Wishing your kid good health. Regards Dr Arun

Brief Answer: Allergy skin test Detailed Answer: Allergy skin test is done by trained allergy asthma specialists. It is a.prick test. If you donot want pricks, you can get Specific IgE Elisa Allergy panel. This is simple blood test. This will be done in major labs in your city Your current prescription does not have budesonide inhaler. Add that , dose mentioned already. Regards Dr Arun
