Question: What do you recommend for a woman who is 83 who has beginning stages of Parkinson's. Best medication, diet, exercise, etc.
Brief Answer:
What are her predominant symptoms-tremors or rigidity?
Detailed Answer:
Hi XXXXXXX m.franke,
Thanks for being on healthcaremagic.com.
I am Dr.Ajay Panwar,a neurologist,here to answer your query.
I would surely like to guide you for the management of parkinson's disease but please tell me beforehand about the symptoms?
Is her Parkinson's tremor predominant or is it rigidity, the chief symptom?Actually, medicines are tailored according to the symptoms.
Is she independent for her daily activities? So,rather than just saying 'early stage parkinson's'-please tell about the clinical details. That will help to formulate a proper management plan.
I shall be glad to have you in follow-up.
Regards

Brief Answer:
Levodopa should be continued.
Detailed Answer:
Hi XXXXXXX m.franke,
Thanks for being in follow-up and providing further details.
Levodopa is the obvious choice as a starting drug while treating Parkinson's disease in elderly, especially at her age.
However,if tremors don't improve much upon Levodopa, Pacitane may have to be added upon.
For now,as she is independent, the approach should be to start Levodopa at a dose of 125 twice a day or thrice a day and observe for the response. This will be the universal approach in this patient.
There are no specific diet restrictions and recommendations.
