Question: What is the best treatment for a twisted knee? Pain at knee joint and above and below the knee.

Dear XXXX,
Thank you for posting your question to HCM.

From what I am understanding you have twisted your knee, and it hurts all around the it.
Now, according to the type of injury, or how long before it had happened, is it swollen and where mostly, is it stable, if you have had previous injuries or interventions in that knee, or if you have problems in other joints...all this information would help determine to giving the best treatment

Meanwhile, for the pain I suggest my patients to take ketoprofen 100 mg 3 times daily. Be careful with that if you are under medications for high blood pressure or other medical problem. In that case take 1000mg paracetamol when needed (but not proven to be much effective)

..A physical exam will decide how severe is the injury.
..If any of the ligaments or the meniscus is teared, will be determined my an MRI (Magnetic resonance imaging) only, which then will decide the best treatment needed. Timing of surgery, if needed may affect outcome in multiple-ligament knee injuries.

--For mild sprains with mild or no instability I would recommend to immobilize knee in splint, use crutches if significant pain and limp, frequent ice for first 48 hours, compressive bandage, pain medications as needed, precautions to avoid reinjury, slow gradual resumption of activity, progressive increase in weight bearing and muscle strengthening (isometric quadriceps exercises) as improvement occurs
--For partial tears - aspiration of bloody effusion, possible arthroscopy, immobilization, padded splint knee immobilizer and circular cast for 3-4 weeks; then vigorous rehabilitation of quadriceps and hamstrings
--For complete tear - surgical repair as soon as possible, best done within 48 hours, possible even 10 days post-injury
