Suggest Treatment For A Right Rotor Cuff Tear

Question: I had a tear in my right rotor cuff.It was a complete tear measuring 14mm AP dimension by approximately 12mm retracted from the footplate I am getting this information from my mri report.Then my left rotor cuff was a high -grade subtotal tear of the anterior corner footplate of the supraspinatus tendon measuring 12mm ap dimension by 11 mm transverse.My left shoulder was done 20 months ago which I have no problems with.My right shoulder was done 14 months ago and is still giving me alot of pain.Especially when i lift something or just move it hurts.I had pt for 6 months on both of my shoulder.I have been seeing a pain doctor I been talking vicodine for the pain right after sugery.I have had 2 shots in my right shoulder which work for about 2 weeks then the pain comes back.I am getting three shots now called tigger points shots I am into my second shot and they have not help so far.I got one more shot to go.The

A complete tear can be repaired surgically but if the tear is an old one or is associated with inflammation then it becomes difficult to heal. The persistent pain in the right shoulder can be due to a persistent inflammation. It is best to get the third shot and see if the pain subsides. Otherwise get an MRI scan done to see how damage is still there which is causing the pain.

Follow up: Dr. Dr. Praveen Tayal (28 minutes later)
I had surgery done on both my left and right rotor cuffs.The tears were about the same size.The same doctor did both surgery and I had Pt on both shoulders for 6 months from the same PT Tech.I cannot figure out why my left shoulder feels fine and my right one still Hurts? You are saying continue with my last trigger point shot .If that does not help go get another MRI of my shoulder to see what is still going on? Should I go back to the Doctor that did my surgery or get a second doctor like yourself to review my problem.If I do get another MRI on my right shoulder what could it show?The doctor repaired the tear put the 4 screws or grafts in it and took care of the bone spur that was their. Would I maybe need another surgery like the that the arscope one or would they do a complete shoulder replacement.Or is it possible like the doctor told me the tear was so large thats the best it will get?

Thanks for writing again.
It is best to get a repeat scan done and go to another orthopedician for a second opinion. The persistent pain can be due to displaced screws also. This needs a radiological evaluation. May be after proper investigations, a repeat arthroscopic surgery can help.
