Suggest Treatment For Abscess Formation Around Broken Tooth

Question: I just got out from my dentist clinic... I've got a treated nerve on one of my teeth, that teeth just broke yesterday (a tiny part, that is resembling a small opening) ... anyway, my doctor reviewed an old "panorama" scan, then he stated that I have a "cyst" or a "pus bag" as good described it, that got into my sinus

He refused to fill that teeth, and got me scared, stating that it will grow and eat my upper jaw bone, causing a hole between my mouth and nose...he said that I should extract the tooth, and go under a surgical procedure to extract it...otherwise the consequences will be bad for my health and sinus.

I asked him to give me a copy of the scan, the scan was taken last July, he said that I need a new scan to determine the extent of the "thing" now and to evaluate what should I do next... I stated that I will submit the scan to another dr. to confirm...
I feel no pain in the broken teeth "since the nerve was pulled out earlier", I just feel a slight movement\small pin touch there...
on the other hand, I have issues with my sinuses (the right one, while the tooth is on the left side, the left side gets blocked easily when I get the flue ), I was just diagnosed with both my eyes tear canal "outer" as blocked, and undergoing treatment for that too.

Can someone please confirm if what my dentist said is true?

As far as I have observed, your broken tooth has abscess (pus) formation due to infection at apex of root which can cause swelling at infected site. As your tooth root is close to sinus so it can also affect sinus if left untreated.

For this preferred treatment is surgical drainage of pus or Extraction (removal of tooth) by oral surgeon.

Consult a good oral surgeon and go for treatment. There is nothing to be worried and don't be scared be relaxed as extraction is very simple easy procedure done by oral surgeon under local anesthesia.

After Extraction you have to take proper course of Medicine and follow post extraction instruction for healing of extraction socket.

Emmm...Actually the picture shows 2 teeth with the abscess...
the other one as he says is far worse and has accessed the sinus cavity.

what affect does this (pus) has on my jaw bone and\or sinuses? How long can this wait? Is it an emergency that should be dealt with immediately as my Dentist stated?

I'm asking this because the procedure as he described is expensive and will take from 2-3 months for me to go through with it.

EDIT: Just to clarify, I feel no real pain so far, no problem eating using any of the teeth, and no inflammation, I'm not sure however if any of the symptoms I'm facing in my sinuses are related to this.

Read your query as you have no pain , no inflamation related to tooth this can be seen in chronic conditions of infection and spread of infection also depends on body immunity .

In your OPG xray(scaned report) contrast is not so cleared so I will suggest you to go for clinical examination and for sinus infection you should go for waters view radiograph or caldwell view confirm the diagnosis.

As you are asking for affect of pus see abscess is infective in nature so it should be treated if left untreated it can cause swelling , bony changes , erosion. So abscess in any point of body needs to be drained.

Dont be worried so as your dentist had described you expensive procedure I will suggest you can consult another oral surgeon or you can approach in government dental college department of oral surgery for treatment and meet head of department for your concern . Hope you will find best treatment there at affordable cost.

My final question (for now) is, can this be avoided during the treatment of both teeth? both teeth were treated and root extracted by the same dentist, more than a year ago. IF it can be avoided by following a more (proper) care, I will make sure to get it right with them.
