Suggest Treatment For Acute Leg Pain

Question: My mother is suffering from acute leg pain from couple of weeks. MRI reports say that there is a herniated disc causing this pain. Please advice on the treatment. Will steroids injection help? What are the side effects of this? Or should we go for endoscopy (spine surgery). What is the cost for this and also side effects? Please help me with this.

Herniation of disc causing sciatica can be managed well.

I have gone through your query. Your problem can be most likely due to sciatica causing from herniated disc. If it is mild to moderate degree then it can be managed conservatively. Muscle relaxants like cyclobenzaprine, NSAIDs like aceclofenac and neurotropics will be helpful for pain relief. Physiotherapy like TENS and muscle strengthening exercises along with this also should be done.

If that is not helping then local corticosteroid injections will be helpful. In case of severe compression of nerve or no improvement endoscopic surgical decompression can be considered.

These surgeries are expensive. It will vary according to different hospital and surgeons. There is possibility of nerve irritation and increasing in pain shortly after surgery.

Corticosteroid injections can have side effects like weight gain, water retention , hot flushes , mood swings and insomnia. There is also risk of dural puncture which can give spinal head ache.

Microdiscectomy or endoscopic correction of herniated disc surgery has 90 to 95 percent success rate. You can go for it if you failed to get improvement from physiotherapy and other conservative measures.

Like any other procedures this also can have complications. Complicatons are nerve root damage , Cerebrospinal fluid leak, Bladder or bowel incontinence, bleeding or infection. But these complications are rare.

There is possibility of reherniation after some months or years after procedure. In such case a revision microdiscectomy can be performed. There are some cases without any improvement after surgery also.

The cost of surgery can vary from 1 to 5 lakh rupees in XXXXXXX In US it ranges from 10000$ to 50000$.
