Can be anal warts though needs biopsy and DRE with proctoscopy
Detailed Answer:
Hi! Welcome to health care magic! Thanks for sharing your concerns with us. We will try to help you in best way possible.
First of all, it is sad to know what you had been experiencing lately. Based on the history you have shared, and the image it can be genital or peri anal warts which can be due to sexual mode of transmission and due to HPV virus.
Definitive diagnosis depends upon, detailed local examination including rectal examination by a dermatologist or a surgeon. Also a biopsy and histopathology may be needed. If it's proven to be warts, cryotherapy or ablation are recommended and more affective than medical therapies like isotretinoin etc.
Wishing you a speedy recovery. If you have any more questions, feel free to ask. Regards.

It can be peri anal skin growth or neoplasm so biopsy is a must
Detailed Answer:
Hi! Thanks for sharing the feedback. Yes it can be skin over growth in peri anal area like in neoplasms or benign though possibility is rare but it should better be biopsies and associated rectal examination and proctoscopy is also advised hence. Regards.
