Suggest Treatment For Attention Deficit Disorder

Question: My son is 4, he is having alot of development issues.
We are currently trying to teach him to count to 10 but he keeps muddleing up numbers, we have tried in fun counteriactive ways but nothing is helping. He used to be able to count to 10 easily a while ago.
Is the same with A stands for apple, apple starts with A, he just doesnt seam to understand it.
He has randomly started another thing that has got me worried, he has two friends tahlia and michel, he has been told there names many times and they have been in his life about a year, every time we go there he calls them ''my friends'. We ask him why he doesnt call them by there names and he just says he doesnt know ir he doesnt remember there names.
He recently got out of hospital, there was a nurse there named XXXXXXX and randomly out of the blue he called her michel. I did not click onto it, we just had a laugh and moved on.
But then the other day we went to go see a very close friend, her name is XXXXXXX But just randomly he called her tahlia.
He does not do that to anyone else, what would make him do that and should i be worried?
That is just the start but there is so much and hoping i could get a little bit if help with this small amount of information.
