The duration of your symptom of having the mass for the last 40 years points to the fact that possibly the mass is a benign lesion.
If I were your doctor, I would after a physical examination of both the testes to see if you have got a normal testicular sensation ( sickening sensation/pain when the testis is pressed ), if it was a cystic or a solid swelling and a general physical examination with special attention to the abdomen, I would advise you for an ultrasound/Doppler ultrasound of both the testes to know the nature of the swelling, and to confirm its consistency.
Whether it was a cystic or a solid swelling, I am afraid that you might need a surgery for a definitive diagnosis by the way of an excision biopsy which could lead you to a cure as well. This would be possible only after a pre anaesthetic check up for you to see your physical fitness for the surgery. I would not advise you for a FNAC in this case.
