Hi.
Read and understood your history.
Noted the history of experimenting with a foreign object, has blood, mucus, black color stool, huge gobs of mucus while urinating and these have stopped. Now there is only mucus in stool and on pushing hard.
Also read about the Pain in rectum, urge to pass but passage of only gas small amount feces and mucus, can not hold.
Less than the truth with your Doctor and could not get the referral to GI but to a Physician, But now there is fever, pain in liver and kidney; want to know the next step.

Can you please tell me what exactly was done?
What was inserted ?
Was there any pain while doing the act?
Or active bleeding at that time?
When did you do this?
Can you move around and change position in bed without added/increase in pain?
I think you should go to the Emergency, tell the truth to the attendant their so that the most important investigations will be done to find out any harm is done or not. Getting a correct diagnosis is very important.
I would suggest you the following and discuss this at the Emergency room so that they do it at an urgent level.
- X-ray of the abdomen in standing position For gas under diaphragm: this is the simplest investigation to give the maximum information.
- CT scan preferably, to see any trauma, collection in the abdomen and so on.
- Colonoscopy is actually not done immediately in the fear to perforate or to spread infection, hence not to be done, it is in fact dangerous, hence not necessary to go to the Gastroenterologist at the moment and getting an appointment may be a problem.
- Tests of blood, urine and stool will help further.
- The most important is the clinical examination by a Doctor who can elicit a classical sign of rebound tenderness and have thorough physical check-up.
Actually in practice: the actual perforation of the rectum or colon is so painful that you can not move at all, you can not talk, can not type. This is one pain which is very classical; if there is no very severe 10/10 pain, do not worry.
Best way is to rush to ER and tell the truth so that you get the best possible diagnosis and treatment.

Thanks for your feedback, it was 3 weeks ago with all the problems you have mentioned.
There may be an internal mucosal injury which has not yet healed.
All you need is a colonoscopy and a course of an antibiotic and metronidazole.
Please get this done and the picture will be clear.
