Yellow dots like pimples with surrounding redness, edema and tenderness indicate secondary bacterial infection of the lesion. This needs early antibiotic treatment to prevent progression of infection and proper healing.

If I was your treating doctor, I will advise you to take antibiotics like amoxicillin+cloxacillin or clavulonate for 5-7 days along with anti-inflammatory agents and antibiotic creams. Kindly see your doctor without delay for starting treatment.

Meanwhile, keep the area clean and dry. Apply antibiotic creams like mupirocin or povidone iodine twice daily. Analgesics like ibuprofen 400 mg thrice daily, taken after food, can help reducing pain and inflammation.

Seeing the picture, it seems like the infection is in the initial stage. There appears no signs of significant deeper infection. Depending on the finding on palpation, sometimes we may just need antibiotic cream and anti inflammatory agents.
Kindly follow your doctor's advice.

Also, it will be fine if you shave the area, so that we can keep the surroundings more clean and dry to reduce bacterial colonization.
