1) The 100 % blocked site requires no treatment
2) When one side is totally occluded other side is 70 % blocked , there are higher chances of stroke in 70 % side , nearly double .
Carotid endarterectomy is a feasible option here . however the perioperative risk is higher .
Chances of death is 4 to 5 % , though it may vary based on different clinical setting .
However even when endarterectomy is risker, it's still a better option than just medical management with improved short and long term outcome .
3) If risk is higher , carotid artery stenting is a very good alternative in his case , do discuss the same with your doctor . just the usage of XXXXXXX should be minimum considering his chronic kidney problem
4 ) By all means he should be put on medication statin and anti platelets forms the corner stone .
