Welcome To HCM. I am Dr Anshul Varshney. I understand your concern.
He has multiple issued including Deep Vein thrombosis (DVT), Stroke and probably drug induced bleeding.
Such cases have to be well managed by Internal medicine specialist as well as Neurologist.
He would require anti coagulation for long term and in acute period with Low Molecular Weight Heparin.
He already has two great risk factors for Stroke and DVT that is Diabetes and Alcoholism.
It's not that these issues can't be managed well at your hospital.
For these issues you can move to higher Center but usually is not required as the treatment is the same as ongoing with Anti platelet drugs, anticoagulation therapy and care of Diabetes.
DVT is however a critical condition as if not treated properly can lead to Pulmonary Embolism that is a life threatening state.
I suppose that the conditions you have mentioned with the medicines are appropriate. So you should discuss with your doctors that they should refer him if they are unable to manage.
If they can manage him here only then I would recommend you not to go for shifting as travel and unnecessary movement of a person having DVT and suspected Pulmonary Embolism should be best avoided in order to avoid the dislodgement of the clot.
