Suggest Treatment For Breathlessness, Tachycardia, Muscle Fatigue And Hearing Loss

Question: I had two teeth extracted roughly two weeks ago....within a day I had marked hearing loss. I tried two doctors before going to the ER two days ago. I reported to the ER with shortness of breath, tachycardia, muscle fatigue and still a hearing loss. All of my white counts are elevated.( have specific if you want ), but ER sent me home. Now, I am still short of breath, muscle soreness/fatigue has worsened, and it is hard for me to remember things. still have a hearing loss. I want to go back to ER but don't want them to send me home again, I want them to fix this, as it is effecting my job. please help

Hi and thanks for the query,

I understand how disturbing this could. However, I think the treatment package that was given you in my opinion is sufficient and good. Adding Ciprofloxacine and prednisolone in my opinion are very appropriate and you might need to be a bit patient so the treatment goes to its end first to permit reevaluation.

I will however suggest you point out to the emergency physician that you might need to get your blood Oxygen saturation measured. In case it is low, this could be an indication to put you on oxygen support for a while to reduce exhaustion.

I understand how disturbing this could be, but the ciprofloxacine and prednisolone given are appropriate in my opinion. IN my opinion, I am optimistic that before the end of the treatment course, you should get really better. Reevaluation by your doctor in case of a very slow evolution might warrant drugs to be given parenterally (injections) rather than orally. This might help. You could suggest this and measuring of Oxygen saturation measurement to your doctor.

However, I am still very optimistic you should be getting better soon.

In your opinion, at what point do I seek follow up care, that time frame? I still have four days left of cipro, no prednisone left however.

Hi and thanks for the update,

I think when you are done with the Cipro, it is important to get dine a new control Complete Blood Count. This shall be very informative on the success of treatment protocol especially on the infectious sight.

If in the next 4 days when the antibiotics are finished there is no improvement with regards to the hearing or soreness, you might have to get a new review. Blood tests like an Erythrocyte Sedimentation and C reactive protein measurements could help give an idea of the degree of inflammation in the body. If values are raised, injections of steroids (fast acting) or increasing their doses for a longer period with caution should be the next step in my opinion.

It is true that even with effectiveness of treatment, response when it comes to regaining hearing and soreness could take some time. However, with the help of the measurements of the above cited markers of inflammation, the decision to change treatment or not can be taken faster.

I suggest calmness till Cipro is finished. then a control complete blood count done. Markers of inflammation to get levels of inflammation and increasing doses of steroids or using injections cautiously for some time could be second options.
