Suggest Treatment For Brown Semen And Constipation Post A Shoulder Surgery

Question: I have discovered recently a brown color in my seamen. This has been happening on and off for the past 3 months or so and usually occurs for about a week and then clears up but then comes back again a few weeks later. The brown color only seems to be in the first amount of seamen that comes out and the remaining seamen appears to be a normal color. This cycle has been happening for 3 months or so now. During this time I have not had any other symptoms or issues related to urinating or pain. The only symptom I have had is slight aching in my lower abdomen (just above my hips) on both sides and lower back. The pain is not much but it causes an ache that lasts for a few hours but also goes away. Would this have something to do with my prostate and should I be alarmed and go see my doctor right away? Or see if it clears up again? I am terrified that the doctor will want to perform a rectal exam as that would make me very uncomfortable. One item to note is that I had shoulder surgery about 5 months ago and during the recovery (about a week later) I had a few days where I was extremely constipated. I had a period of several hours where I was on the toilet trying to push feces out but it would not break off therefore I had it somewhat out of me but I would have to give up and the feces would go back inside my behind because it was so hard. I attempted to push for a few hours trying not to damage anything in my rectum until stool softeners kicked in and allowed me to pass it. Would this type of constipation cause a possible infection or damage to my prostate?
Other information is that I am 29 years old and married with the same partner for 9 years. Sexually active maybe once to twice a month. Any help or advice would be appreciated! Thank you

The symptom that you describe (brown discharge in your semen) in fact correlates with a problem of your prostate.

It might be:
- prostatitis (infection/inflamation of your prostate)
-stones in your seminal vesicles or prostate
-polyps in the urethra
-ejaculation duct obstructions
-cysts
-infection or inflamation of your testicle
-benign or malign tumors of prostate

In most of the cases, brown discharge (porbably it is blood) the reasons are benign.

If i was your caring doctor i would recomend these tests: urine test and urine culture too see if there is any infection in your urinary tract and to treat it.
-ultrasound of your genito urinary area (testicles,bladder,kidney ect)
Depending from these results you might need or not to consult an urologist.

Digital rectal exam is one of the best exams to check your prostate. If you feel uncormfotable for this procedure you will need to talk with the urologist if there is any possibility to avoid this exam and do other tests like MRI.

Contipation will not afect directly your prostate,but if the prostate is enlarged if can increase the amount of brown discharged with your semen.

Before starting to be worried,i suggest to do the tests that i mentioned ahead and depending from the tests,take the proper treatment.

Thank you for your detailed answer. Just to prepare for my appointment with my doctor, would you assume they would possibly perform a digital rectal exam right away or only the urologist would do so if it gets to a point where I am referred to one? Thanks again for your help! I will make an appointment today.

Every doctor is able to do a digital rectal exam to check your prostate. BUT an urologist can feel even the smallest changes of the prostate.
I don't know if your doctor will perform a digital rectal exam or he will prefere to let the urologist do it.

After the tests are done,your doctor would be able to tell if you need to do a digital rectal exam or not.

There are a lof of cases when the doctor doesn't need to do that exam,because he is able to have a diagnosis according to the results of the tests.
