I have had a had a brown spot on the bottom of my toe for some years. Should I be concerned?

I have read your query.
I understand your concern.
I have seen the attached photograph also.
As per details, it seems that you are having nevi on the foot. Some nevi are present since birth and some develop afterwards in life.
It is totally asymptomatic and harmless.
But in case of sudden change in size, shape, pigmentation or development of symptoms like itch or pain , it needs to be evaluated.
You need not to worry for this. Still if you wish you may go for Skin biopsy to confirm the diagnosis .
You should visit your doctor in person so that he can examine you and carry out clinical examination. He may order Skin Biopsy for you.
You can share my opinion with him.
