Hello ,
I have gone through the details you have provided,
Usually Symptoms of reflux are better after six to eight weeks of therapy.
In casr your symptoms persist beyond 8 weeks then you need further investigations to find out why your reflux symptoms are not responding to treatment.
As your symptoms have not completely disappeared after 40 mg of nexium the next step would be to get an upper GI endoscopy done to check for
erosive gastroesophageal reflux disease, hiatus hernia , gastritis and H pylori infection and any other gastric or esophageal lesion.
The further line of management can be decided after you get an upper GI endoscopy
