Any chest pain must be immediately investigated to rule out impending life threatening events such as a heart attack. The same goes for severe headaches with unknown cause.
-Do you have hypertension, diabetes, Thyroid hormone imbalances or any other medical problems other than what you mentioned?
Your symptoms would lead me to primarily rule out atherosclerosis of the coronary vessels (heart) and cerebral vessels (brain)
I recommend :
-that you visit the ER right away
- Have an electrocardiogram, echocardiogram, carotid doppler, MRI of the brain (MRA & MRV - of the blood vessels of the brain included)
-blood tests (complete blood count, Thyroid profile, kidney & liver function tests, cholesterol and blood glucose levels)
Based on the findings of your clinical exam & the results of the above tests, diagnosis and treatment can be initiated.
It is of utmost importance to start treatment at the earliest in case of a clot/atherosclerosis as every minute counts towards a quicker & complete recovery.
I don't mean to alarm but to caution you to the possibility of events which can become life threatening quickly, especially so with a strong family history of these diseases.
Avoid exercise or straining. Have someone with you at all times, at least till you see a doctor
