Hi.
Thanks for your query, albeit short and specific.
To recapitulate: Female/64 - chronic anal itching - black specks in stool - constant diarrhea - trying to keep area very clean.
As per the short history that you have given the apparent sequence looks to be as follows:
- It might have been started as intestinal infection causing diarrhea and black specks may be either due to the blood secondary to infection. Once the stool comes out the infection in the peri-anal skin causes skin excoriation and itching.

This can be diagnosed by:
- Tests of stool for three consecutive days.
- Tests of blood: CBC, Thyroid, Sugar and as guided by clinical evaluation.
- Per-rectal examination, proctoscopy and colonoscopy and biopsy will help further to identify the cause and know the effects of the treatment.
- A course of an antibiotic, metronidazole, probiotics can help alleviate the problem.
- Multivitamins and particularly Vitamin A and D in therapeutic dosages if there is deficiency as seen by the blood tests. Helps in early and better healing.
- Treatment of any medical condition if found out.
Sitz bath in a tub with warm water for local soothing and cleaning with the same water.
