default
Posted on Wed, 4 Feb 2015
Question: I have chronic migranes, sometimes two to three per week. I have tried everything from seven different neurologist. I have been attending a pain clinic but they said they could not treame again because my urine is clear. Nothing ever shows up. They had me on oxycodine 30 6 times a day and it helped but as you know the body builds up a resistance so I need to increase the strength or change to something like dimeral. Which is what i get when I go the ER.

Brief Answer:
Migraine requires medical as well as general care.

Detailed Answer:
Hello,

My name is Dr Muhammad Ali Rauf & I will look into your problem.

I have gone through your query and would try to help you in the best possible way. In any disease there is general care as well as medical care. As you are already taking strong dose of opioids so you need to consider general care as well. I would suggest you to discuss the use of sumatriptan, propanolol & amitriptyline with your treating doctor. These drugs are prescription drugs so you need to visit hospital.

Now the important thing is to prevent those factors which will aggravate the migraine. If I were your treating doctor then I would have advised following general measures:

1. Diet:
Following food aggravates symptoms:
> Alcohol
> Caffeine overuse
> Chocolates
> Citrus Food & banana
> Dairy cheese
> Vinegar

2. Stress:
Stress is a potent stimulator of headache. If you have past history of stress related issues then you need to visit hospital to get medication for it as they are prescription only.

3. Exercise:
Try to have a walk in the evening for 30-45 mins. Change in environment will help to reduce stress & improve health.

4. Sleep:
You mentioned that you work straight hours and in front of monitor, so it will be playing a major role in aggravating your headache. You need to manage adequate sleep of at least 5-7 hrs/day.

5. Light:
Flashing lights like in clubs also stimulates the headache so you need to avoid them.

6. Keep a Headache Diary:
It can help you keeping the record of triggers. Write down everything you eat and drink, your exercise routine and schedule, the weather, strong feelings and emotions you may be having, your medications and their effects, and times and severity of your headaches. This can help you see a pattern in your migraine occurrences, and will make avoiding one easier.

7. Hormonal Changes:
Particularly important for females as it may aggravate the headache, specially oral contraceptive pills.

8. Weather may also effect:
Humidity and hot temperatures can stimulate headaches, as can rainy days, so it’s important to be aware of your environment.

Hope I have answered your query. If you have any further questions I will be happy to help if not then don't forget to close the discussion and rate it.
Wishing you good health !!

Brief Answer:
You may start by discussing with your doctor

Detailed Answer:
Hello

Thank you for getting back with the query. codeine and its derivatives oxycodone are termed as opoids. Oxycodone, a semi-synthetic opioid which as you mentioned are already taking at a dose of 30mg 6 times a day. If you have stopped it then you can discuss with your local doctor for giving you prescription again. Opoids are given for severe migraine. As your symptoms reoccur frequently in a week and more severe so it would be better option. Its a prescription medicine so i can not prescribe you here. You can also discuss with him to give you combination of medicines(opoids+beta blockers) to prevent resistance and for effective results. BUT as i mentioned earlier, you need to take general care as well so that medicine can produce effective results.

Hope I have answered your query. If you have any further questions I will be happy to help if not then don't forget to close the discussion and rate it.
Wishing you good health !!
