Hi,
Thank you for posting the question.
I have gone through your medical history in detail.
If you have full strength and range of motion,then there is less possibility of a complete infraspinatus tear. It may be a grade 1 strain that should heal up spontaneously.
Such strains do take a lot of time to heal.
If it becomes a chronic tendinitis (inflammation of the tendon),then physiotherapy,Short wave diathermy or LASER therapy may help.
As a last resort,a local steroid injection around the tendon can provide much relief.
In my opinion,most probably,surgery is not needed at this stage.
(Only an MRI can surely decide about the need of surgery).
