Question: What causes a cramp like, consistent abdominal pain? This happens almost regularly every 4 - 6 weeks. My daughter is 12 years old now but when she was younger she would throw up and wretch really hard even when there was nothing in her stomach; these episodes would recur almost every 6 weeks.

Noted the history about a 12 year old girl having cramp like consistent abdominal pain - almost regularly every 4 to 6 weeks - would throw up and wretch really hard when there was nothing in her stomach when she was younger and these episodes would recur almost every 6 weeks...

Please give some additional information for me to assist you better:
Has she started her menstruation - menarche (first occurrence of menstruation)?
Any other symptoms during the attack like chest pain, breast pain?

Since the pain/vomiting/wretching has the cyclical nature and occurring every 4 to 6 weeks, it is possibly related to the hormonal cycle that is expected to/might have have started as your daughter's age is 12.

Awaiting for your feedback so that we can discuss further. If this is related to the cyclical changes there is need to have Ultrasound/MRI of the whole abdomen, particularly pelvic area and an opinion of a Gynecologist.

During the time when she experienced the wretch ing her doctor tab the time suggested it maybe 'migraine' manifesting itself in vomiting.???
She has started menstration but does not complain during the cycle.

Please get the Ultrasound/MRI of the whole abdomen, particularly pelvic area and an opinion of a Gynecologist particularly when she is in pain.
Getting the investigations during the pain is very important - not to be done when not in pain.
