Brief Answer:
The bacterial infection should be ruled out

Detailed Answer:
Hello
Thanks for the query

Fever with cough in the presence of increased neutrophil in blood is suggestive of any bacterial infection.
There can be various causes of presence of ketone in urine. In case of fever or starvation/persistent vomiting ketone can come in urine.Sometimes urinary ketone also indicates diabetes.A random blood sugar should be done to rule out diabetes and if it comes normal,a repeat urine test after 3 days should be done to see whether ketone is still coming.
I would like to rule out any bacterial infection(streptococcus is a common cause)by doing ASO titre, strep test and CRP.
Antibiotic treatment is needed in bacterial infection.
So I would suggest you to consult a physician for proper evaluation and management accordingly.
I would be glad to answer if you have any further query
Regards
