Suggest Treatment For Depression In A Bipolar Person

Question: A psychiatrist put my bipolar daughter on regret all and she went into the worse depression ever I wanted him to take her off it two weeks after she started This doctor said regret all does not cause depression and he is head of Harford Belair mental health center Then he told me to prove it and I rough him many souraces Then he added Wellbrutrin.Then last week her blood work showed her neurifil's were dangerously low Then he said you can stop it and go on depakote Hehas a bad memory and did not look in her records because he took her off depakote and put her on regrate 2 and a half moths ago because depakote a used awful side effects She is on a antipyretic and neurotic as well SO MT QUESTION CAN SHE STOP TEGRETAL AND START LAMICTAL SAME DAY.I AM DESPERATE THE DOCTOR BEFORE HIM 2 YEARS AGO TOOK HER OFF HER ANTIPSYCHOTICBECASESHE HAD NOT HAD A EPISODE IN 8 YEARS IT CAUSED SEVERE DEDUCTION TO HER LIFE SHE REFUSED TO BELI'VE SHE WAS SERIOUSLY UN WELL.IT WAS A NIGHT MARE FINALLY SHE MADE THE BADDECISION TO GO OFF HER LITHIUM AS WELL AND WENT INTO FULL BLOWN PSYCHOSIS AND I HAD TO GET THE POLICE TO TAKE HER TO THE HOSPITAL IT SAVED HER LIFE.THE DOCTORS IN XXXXXXX MD HAVE BEEN DANGEROUS TO HER LIFE

I read your query and understand your concerns and anguish against the health services.

Before I proceed to answer I assume that the mentioned "regret all" in your query is tegretol.

Regarding your question, In my view it should not be done in abrupt manner. Both Tegretol as well as Lamictal known to cause Stevens XXXXXXX syndrome and I do not think talking risk if warranted. If we use them together there is high possibility of this serious side effects and we need to have some gap between these two medicines.

What I feel that you stop the tegretol all together and wait for three to four days to get it flush out from the system. This is the minimum time provided she is using tegretol for months. The half life of tegretol on prolonged use is around 16 hours and it takes five half lives to get it cleared from the system.

After four days of tegretol discontinuation you can start Lamictal at lowest possible dose, say 25 mg per day and increase it gradually as decided by her doctor.

It's not possible for my daughter to taper off Tegretal and not be on some ting at all times She has severe bipolar and would become full blown manic etc She is scared to death to do something like that.She has been thru horrific psychosis at the hands of truly Uncaring doctors.So can you come up with another solution

Thanks for reverting back to me.

I understand your concern as the full blown mania is another scaring thing alike stevens jobnson syndrome. In this situation I think we need to think for another option which have no or minimal interaction with tegretol.

Considering her condition I feel Seroquel can be started at the dose of 50 mg and to be increased to 100 mg on day three and 150 mg on day seven. Following this tegretol can be stopped all together.

Since seroquel is prescription only medication you need to discuss same with her treating doctor.
