Question: I masturbate a lot and even though I clean it after the activity, there seems to have developed a rough thick layer of semen next to the head on the edge. This causes a lot of discomfort during sexual activity. It stays itchy and hurts when I pull back the foreskin. There is also a layer of semen on the head that does not go away after a shower too. Need urgent help

I am dr.kakkar (dermatologist). I have gone through your query and i have noted your concern.
You seem to have developed a sort of dermatitis as i can see in this image, may be due to friction
i suggest you to use a topical moderately potent steroid and antibiotic combination ointment e.g fluitcasone propionate 0.05% and 2% mupirocin ointment (Flutibact ointment), twice daily for a few days.
