If he is not having features like increased thirst and urine output, weakness, tachypnea, nausea, drowsiness etc (suggestive of keto acidosis), need not be taken as an emergency. But, treatment should be reviewed without delay at the very next appointment, preferably next day. This is unlikely to be controlled by metformin alone. We may need to add other hypoglycemic agents or insulin.Your doctor will guide in this.
Effect on kidney / eye is long term effect;not acute effect. It depends on poor glucose control over a long period.
Meanwhile, ask him to cut down calorie and fat intake. Try regular moderate exercise daily.
