Question: Hello Doctor, I am suffering with PCOD since last 4 years. Even in my childhood i have suffered with irregular periods. Now Its been 4 years i got married and now since last 1 year im planning for pregnancy. i am using cetaphin 2000 mg in a day now and my doc told that the ovaries are normal in size. And i am taking the medication Enclova 100 mg & Pubergen 5000 & Duphaston for a proper regular cycle, increase in follicles & for ovulation but only once, in the month of march 2015 i succeeded in ovulation but the result was negative. My doubt is upto how many more days i should continue this medication and what is the next protocol if i dont conceive with this medication process? Im undergoing this treatment since last 6 months.Kindly suggest as iam eagerly wanting to have a baby before i turn 30 yrs of my age. My weight is 78 kgs and height is 5'2

Brief Answer:
May need a change over of medication.

Detailed Answer:
Hi Ms. XXXX,

Thanks for writing to me.

There is no definite benefit of using Enclomiphene over Clomiphene in endometrial formation, but, it can induce multiple follicle generation. The fact that you have responded to treatment only once during this phase calls for a change over in the medication.

You may need to raise the dose of Clomiphene until ovulation is achieved. The maximum possible dose is 200 mg. and for a maximum period of six months.

Since you have had no regular ovulation with the current dosage, you may try with altered medication for another three months. If there is no further response, you may need to try IVF or other possible alternatives like donor egg fertilization.

You are overweight, so please try to lose weight by diet modification and regular exercise. This also helps to regularize your cycles and improve your response to medication. At your height, you can weigh a maximum of 56-58 kg. The normal range of BMI is 19-25.

Hope your query has been answered. Please feel free to contact for further clarifications. I will be happy to help.

regards,
Shanti.V.
