Retracted and swollen foreskin suggest inflammation of foreskin.It is also called balanitis.
Considering your history likely cause in your case is injury during sex or excessive irritation of foreskin during sex. Sometime condom may be allergic to foreskin.
Other causes include infections which is mostly fungal, allergy to soap used to clean the foreskin or application of lubricant or cream for sex,and poor hygiene.Just think about any of the factors in your case.

I can understand your concern. Your situation is called paraphimosis. Swelling is moderate. It can come back to normal.
Apply ice packs after wrapping penis in plastic.Gradually try to retract foreskin manually if swelling decreases. If you can not bring foreskin forward over glans today , consult doctor tomorrow.If today it is too painful, consult doctor now. Avoid sex for few days.
More the compression of blood vessels more the swelling and pain, which can restrict blood supply to glans penis.
It can be manually retracted by doctor after local application of mannitol or lignocaine.Or minor surgical procedure called puncture technique can be used.
