Question: I have a pace maker, and I got real dizzy and a little nauseated earlier and then when I took my blood pressure it was 96 /55 and my pulse was 60. I am not having any chest pains, and bp is now 101/67 pulse 60. Am I safe to stay home or should I go to an ER?

I would explain as follows:

Detailed Answer:
Hello,

Welcome on HCM!

Regarding your concern, I would explain that your actual complaints seem to be due to hypotension.

As you have a pacemaker implanted, you should feel safe about your heart rate (pacemaker doesn't allow decreasing heart rate below a certain value [60 bpm]).

For the moment you shouldn't worry too much. Just take plenty of water and fluids, avoid hydrochlorothiazide next morning.

You need to closely monitor your blood pressure values (several times daily for a couple of days).

If hypotension trend persist, I recommend consulting with your cardiologist for a possible therapy optimization (reduce sotalol daily dose, avoid hydrochlorothiazide and check the actual thyroid function).

Hope to have been helpful to you!

In case of further uncertainties, do not hesitate to ask me.

Kind regards,

Dr. Iliri
