Suggest Treatment For Epilepsy
My daughter Ms. XXXX 11/F who is under your treatment for absence seizure- epilepsy, had undergone a MRI brain scan - epilepsy protocol and EEG as advised by you. Though the EEG report was normal, her MRI brain scan showed Mildly elevated T2 relaxation times in the hippocampi bilaterally. As per report, you had prescribed her the following medicines with a review in November 2014;
1. Tab Valparin Chrorno 300 mg (Twice a day) - for 3 months
2. Tab Oxetol 150 mg (Twice a day) - for 3 months
As per your advise we had administered the above medicines continuously on daily basis. During this period she had not suffered any further attacks of absence seizure and seems normal. She is attending her school regularly.
You are requested to kindly advise us future course of action in this regard.
Thanking you,
Yours faithfully,
XXXX

Same medicines may be continued.
Detailed Answer:
Thank you for getting back.
I am glad to note that XXXXXXX is doing well on medications, and has remained seizure-free.
At this point in time, same medications may be continued at the same dose.
However, just to make sure, that there are no side effects of medications, please get the following blood tests done:
1. Liver function tests (LFT)
2. Serum sodium level.
Once the blood reports are ready, you can upload the reports here.
I hope my answer helps. Please get back if you have any follow up queries or if you require any additional information.

Thank you for your guidance. As you have mentioned "side effect of medicine" I would like to inform you that lately she has been complaining of hair fall. Could this be a possible side effect of medicine ?
I would in the meanwhile get the tests done and have them uploaded for your perusal.
Thank you,
XXXX
Yes, hair fall can occur with medicine.
Detailed Answer:
Thank you for getting back.
Hair fall can occur with the use of Valparin chrono tablets. To reduce this side effect, one vitamin CORUS (made by Intas company) may be used, one tablets daily in morning.

I am very sorry for the inconvenience caused due to my oversight. I have now uploaded the correct report of LFT & Serum Sodium Level for your perusal and advice. You are requested to kindly intimate how long the prescribed medication needs to be administered so that I can plan accordingly.
I once again apologise for wasting your valuable time.

Liver enzymes are slightly high.
Detailed Answer:
Thank you for getting back.
Liver enzymes are slightly high.
Please reduce the dose of valparin chrono to 200 mg two times daily (instead of 300 mg two times daily). Discuss about this with your doctor.
Continue Oxetol same dose.
Recheck LFT after 10 days and inform me.
Best wishes,
Dr Sudhir Kumar MD DM (Neurology)
