Suggest Treatment For Erectile Dysfunction After A Urethroplasty
Question: I had an end to end urethroplasty completed Nov 23rd. My surgeon spent very little time of the erectile side effects. It appears that my erections only point down or to the side and are also about 1.5 inches smaller than previous. I am less concerned with size, the direction is very bothersome. Is this something that should improve over time?

Thanks for your query ,based on the facts that you have posted it appears that you have undergone surgery of End to End anastomotic Urothroplasty for stricture urethra
I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Was stricture traumatic or infective in origin ?
2) What was the site of stricture ? was it in Bulbar Urethra or Penile urethra .?
3) Do you get good erection hard enough to have penetrative intra vaginal sex ?
4) Is it possible to upload photograph of your penis both in flaccid sate and after erection ?
5) Are you able to penetrate vagina and can perform sexual intercourse ?
Please provide details with photographs so as to help me to make precise comments and suggestion to resolve your problem .

Here are the answers to your questions......
1) Unknown, the stricture has been managed for 25 years.....but my guess was infective originally
2) Unsure, however my incision was across the perineum and scrotum. My flaccid penis is 3-4 inches and erect (was) 6.5 - 7 inches (now about 1.5 inches smaller). The stricture was located 4 inches form the tip (I know this from when I self catheterized)
3) Yes....although uncomfortable due to angle. It is comfortable when pointing to to either side but also curves down (like a banana). It is very uncomfortable if forced up in the normal position
4) Not now, as I am at work and using work technology
5) Yes
It seems hard to find much on line in terms of the degree of trauma and hence whether this is a permanent situation or something that would improve over time. Clearly this is very disturbing

You had a stricture of Bulbar Urethra of a length of 1.5 cm which was excised and end to end anastomosis of urethra was done .The curvature of the penis downward on erection may be due to shortening of urethra resulting out of excision of a strictured segment .
The curvature of penis downwards is unlikely to get corrected itself .
If the curvature is imposing you difficulty in penetration of vagina I t can be corrected by a surgery of Placation of Tunica Albugenia (sheath that covers erectile bodies of Corpora Cavernosa) on dorsal side.
As regards your query about the length of urethra I would sytate that female vagina on arousal is 4 inches in length and capacious enough to accommodate erected penis of 4-5 inches without pain .Since you have 5 inches long penis on erection you may not bother about it as long as you have enjoyable sex .
