Brief Answer:
Need more information from you.

Detailed Answer:
Hello,
Welcome to HCM.

I have read your query and understand the concern. So, you are facing problem of erectile dysfunction since 4 years and low sexual desire since few days.

Erectile dysfunction in an otherwise young adult usually has no major organic cause, and often responds well to drugs belonging to a group of drugs called 'PDE5 Inhibitors', most noted of which is Viagra.

However, I would require some more information to understand your condition better and before suggesting any medicines (like tentex forte) since you are a bit afraid or hesitant to use any of these tablets (Viagra).

Kindly give a few more basic details,

1) Regarding your libido which means desire to have intercourse,

2) Do you have nocturnal penile erections when you sleep, especially in the early mornings?

3) Also, if you can quantify the degree of dysfunction on a scale of 1-10, it would be useful to assess your condition.

4) How was previous sex life or married life?

5) How did the problem start? Any specific incidence was there?

6) Do you pay more attention to erection while trying for intercourse?

7) Do you have any physical illness, specifically diabetes or high blood pressure or are you taking any medications?

8) Is there any history of drinking alcohol since long time apart from smoking?

9) How is relationship with your partner? Any emotional or social problem is there?

Kindly get back to me with this more information. I will be happy to help you after getting this answers from you.

Awaiting your response.

Brief Answer:
Needs further evaluation.

Detailed Answer:
Hello,
Welcome again and thanks for sharing more information.

As per your fact that you have answered here, it appears that you have problem of getting good erection while attempting for sexual intercourse is because of psychological, stress or some underlying pathology but it must be further investigated by various tests.

However, certain measures will help you to boost up your confidence and may help in getting good erection and have enjoyable sex.

1) Do regular exercises for about 30-45 minutes for half an hour in the morning that helps in improving concentration & builds up the confidence level. Avoid stress.

2) Take balanced protein diet (rich in diet rich in fruits, vegetables, fish, nuts & honey and also take vitamin A,C,D,E and Zinc).

3) Take anti oxidants like Almonds 5-6 everyday.

4) Avoid alcohol (even social drinking) completely. These can narrow blood vessels of penis and have been linked to erectile issue.

5) Have adequate sunlight exposure in daytime which will decrease this melatonin level and in turn will help to get sexual desire.

6) Paying more attention to your partner’s desires and talking about this before hand can make sex pleasurable for both of you.

7) Engaging more in foreplay with your partner before intercourse.

8) Ayurvedic medicines 'tentex forte' can also be used for to maintain erection during sex and also they are without any side effects.

9) Another medicine cum vitamin supplement containing pyridoxal-5-phosphate, mecobalamin and l-methylfolate (trade name tetrafol plus in India) is also very helpful for getting good erection.

10) Here,I can understand that you are a bit reluctant to take Sildenafil citrate 50 mg (Viagra) but it may also be a good option and have lesser side effects on 50 mg dosage. You will need prescription from some local physician for it.

We may expect that you will see improvement in getting and maintaining good erection within 6 to 8 weeks with above measures and if still there is no change,it would be better to rule out certain medical causes like diabetes, hormonal problems, blood flow problems to the penis, vitamin deficiencies, etc.

Also, consult urologist for further treatment of your progressive glomerulonephritis although it is unlikely to be linked with erectile issue.

Please let me know if you need any more clarification further. I will be happy to answer that.
